Heart health reviews (for existing conditions)

We offer regular cardiovascular (heart health) reviews for patients with known heart disease or related conditions. You will be asked to attend for a blood test first, so we can review your results alongside your appointment.

Your review is then carried out by one of our Healthcare Assistants (HCA) or Practice Nurses and will include checks such as blood pressure, cholesterol, and lifestyle factors to ensure everything is working as it should.

Regular monitoring is important — heart and circulatory diseases are one of the leading causes of illness in the UK, but good ongoing management can significantly reduce the risk of complications such as heart attacks and strokes.

You will usually be invited for review at regular intervals, depending on your condition.

Heart health checks (prevention)

We offer heart health checks to help identify your risk of developing conditions such as heart disease, stroke, diabetes, and kidney disease.

These checks are typically offered to patients aged 40–74 who do not already have a diagnosed heart or circulatory condition. The appointment may include a blood test, blood pressure check, and a discussion about your lifestyle and family history.

The aim is early identification — many heart and circulatory conditions develop quietly over time, and simple changes or treatment can significantly reduce your risk. In fact, early intervention can prevent a large proportion of heart attacks and strokes.

If you are eligible, you will usually be invited for a check every 5 years. If you are concerned about your risk or would like to discuss this sooner, please contact the practice.
