We provide ongoing care and monitoring for patients with long term conditions (sometimes called chronic diseases). These are conditions that require regular review and support to help you stay as well as possible.
This includes (but is not limited to):
- Diabetes
- Asthma
- COPD
- Heart disease
- High blood pressure
- Chronic kidney disease
- Stroke or TIA
- Epilepsy
- Overactive Thyroid
- Underactive Thyroid
- Mental Health
You may notice that not every condition has its own section on our website. Please don’t worry — we have systems in place to make sure all of our patients with long term conditions are reviewed regularly.
We will usually contact you when your review is due, but if you feel you are overdue, or if it would simply put your mind at rest, please do get in touch. You really won’t be bothering us — we would much rather hear from you and not at home wondering.
